An essential part of the service that we provide to organisations is to be able to directly demonstrate how the work they do has an impact on National Outcome Frameworks. As the policy environment changes then we can easily incorporate new developments into our Risk Tracker system without any inconvenience to the people using it to monitor clients.
We see the CCG Improvement & Assessment Framework 2016/17 as a key document that will govern how commissioners design and monitor services in the future. We have taken the fifty seven indicators from the Framework and mapped them against the common issues that we already monitor in day to day services.
This has meant that we’ve been able to update the data dictionaries that sit under our risk maps for:-
- Starting Well
- Developing Well
- Working Well
- Ageing Well
- End of Life
- Diabetes
- Substance Misuse
- Supported Housing
All of these service areas have an impact on the objectives set out in the Improvement Framework, we can now help organisations set out to their commissioners how they are helping to meet them.
